At Keystone Hospice, we understand how stressful this process can be. That’s why we are dedicated to making our hospice care admission process as smooth and hassle-free as possible.
Once we receive a referral, a registered nurse and social worker will visit you or your loved one and family in the home. We’ll conduct an evaluation, fully explain our services, answer questions, address concerns and apply the benefit of years of experience helping patients and families make a decision about hospice care. We also discuss with the treating doctor to verify that hospice care is the correct choice. Once primary care physician agrees with treatment, hospice care can begin almost immediately, generally in four hours or less.
We also offer an urgent admission option to those with critical care needs. A registered nurse, social worker and any other member of the Keystone Hospice team can be at your home within two hours of receiving a physician’s order.
